The Kidner procedure is a surgical treatment for a painful accessory navicular, which is an extra bone on the inner side of the midfoot. It addresses irritation where the posterior tibial tendon attaches to this bone, leading to arch pain, swelling, and difficulty with activity. The goal is to remove the painful extra bone and restore the tendon’s ability to support the arch. This procedure focuses on bone and tendon structures in the foot. Your surgeon may recommend it when bracing, orthotics, rest, and physical therapy have not relieved symptoms. At OrthoNJ, we work with you to decide if this option fits your goals and lifestyle.
During surgery, the surgeon makes a small incision along the inside of your foot, removes the accessory navicular, and reattaches or advances the posterior tibial tendon to healthy bone using strong stitches or suture anchors. This helps eliminate the source of rubbing and restores the tendon’s pull to support the arch. The procedure is commonly performed as outpatient surgery, and careful technique is used to limit soft tissue irritation and discomfort.
After surgery, your foot is protected in a splint, cast, or boot, and you will avoid putting weight on the foot until your surgeon advises. Swelling and soreness improve over several weeks, followed by physical therapy to restore motion, strength, and balance. Many people transition to regular shoes and daily activities over a few months, while return to higher impact activities can take longer. Recovery time varies based on your health, the severity of tendon irritation, and how closely you follow your rehabilitation plan.
