Hammertoe fixation is a surgical treatment that straightens a toe that stays bent at the middle joint. The deformity involves the small bones, joints, tendons, and supporting ligaments of the lesser toes, which can lead to painful corns and shoe irritation. The goal is to realign the toe so it rests flat and functions more comfortably during standing and walking. We consider this option when splints, pads, wider shoes, and other nonoperative care no longer control pain, or when the toe becomes stiff and difficult to correct. At OrthoNJ, we help you decide if surgery is appropriate based on your symptoms, activity goals, and exam findings.
During the procedure, your surgeon releases tight soft tissues and realigns the toe’s small joint, then holds the bones in the corrected position with a pin, screw, or low-profile implant while they heal. Many cases can be done through small incisions to limit disruption to nearby tissue, which may reduce soreness and help you return to your normal activities sooner. The specific technique depends on whether the toe is flexible or rigid and whether more than one toe is involved.
After surgery, you will typically wear a protective dressing and a stiff-soled postoperative shoe or boot while keeping the foot elevated to limit swelling. Limited weight bearing is common at first, with a gradual return to normal shoes and activities over several weeks to months, sometimes after removal of a temporary pin in the clinic. Physical therapy may be recommended to restore motion and balance, and recovery can take longer if multiple toes are corrected or if you have factors such as diabetes, smoking history, or circulation problems.
