Ankle ligament reconstruction is a surgical procedure that repairs or replaces stretched or torn ligaments that stabilize the ankle joint. It is recommended for chronic ankle instability, repeated sprains, or persistent pain and giving way that do not improve with bracing, activity changes, or physical therapy. The procedure targets the ligaments and surrounding soft tissues that keep the ankle bones aligned during motion. By restoring tension and support, it aims to reduce sprains and improve confidence in walking, work, and sports. At OrthoNJ, we consider this option when instability limits your daily life or prevents a safe return to activity.
During surgery, the surgeon tightens and repairs the damaged ligaments using strong sutures and small bone anchors and may reconstruct them with a small tendon graft from your leg or donor tissue when the native ligament is too weak. Many cases can be addressed through small incisions and, when appropriate, ankle arthroscopy to assess and treat joint problems. Using limited incisions helps protect nearby tissue, which may decrease pain and support a smoother recovery.
Recovery begins with a period of immobilization in a splint or boot, often with crutches and limited weight bearing, followed by gradual progression as healing allows. Physical therapy focuses on ankle motion, strength, balance, and proprioception so you can return to work and sport safely. Many people resume regular daily activities in about 8 to 12 weeks, while full return to running and cutting sports can take 4 to 6 months depending on age, tissue quality, associated injuries, and adherence to the rehab plan.
