First MTP Joint Arthrodesis

First MTP joint arthrodesis is a fusion surgery for the big toe joint, where the first metatarsal bone meets the base of the toe. It is used to treat painful arthritis, severe bunion deformity, instability, or damage that has not improved with nonsurgical care. The procedure removes the worn cartilage and permanently joins the bones so the joint no longer grinds or shifts. This targets the bones and joint surfaces and can also help nearby soft tissues by restoring alignment. At OrthoNJ, we recommend fusion when pain limits walking, shoe wear, or work despite changes in footwear, orthotics, medications, or injections.

What First MTP Joint Arthrodesis Entails

Through an incision over the big toe, the surgeon prepares the joint surfaces by removing remaining cartilage and shaping the bones so they fit together in a corrected position. The bones are held securely with low-profile hardware such as screws and a plate, and bone graft may be used if needed to encourage fusion. This stabilization stops painful motion and realigns the toe, which can reduce irritation, improve push-off, and protect surrounding tissues.

First MTP Joint Arthrodesis Recovery

Recovery often includes a period of immobilization with the foot protected in a cast or boot and a gradual return to weight bearing as the bones fuse. Swelling and stiffness improve over weeks to months, and a guided rehabilitation plan focuses on walking mechanics, balance, and ankle and lesser toe motion. Most people resume regular daily activities in several weeks, while full recovery and return to higher-impact tasks can take a few months depending on healing, overall health, and adherence to post-operative instructions.
