Posterior tibial tendon reconstruction with calcaneal osteotomy is a combined foot and ankle surgery that restores the arch and relieves pain caused by posterior tibial tendon dysfunction, often referred to as adult acquired flatfoot. The posterior tibial tendon supports the arch and helps to turn the foot inward; when it weakens or tears, the arch collapses and the heel drifts outward. The procedure addresses both the soft tissues, such as the damaged tendon, and the heel bone known as the calcaneus. By repairing or reconstructing the tendon and realigning the heel bone, the surgery aims to improve walking, balance, and comfort in footwear. During surgery, the damaged portion of the posterior tibial tendon is cleaned and repaired, or the tendon is reconstructed, and the heel bone is repositioned with a precise cut so it can be shifted to a better alignment and secured with small implants. These steps rebalance the pull of the muscles, restore the arch, and reduce stress on the inside of the ankle. Recovery typically includes a period of immobilization and no weight bearing in a cast or boot, followed by gradual weight bearing and physical therapy to restore motion, strength, and balance. Timelines can vary based on the severity of the deformity, your overall health, and whether additional procedures were performed.
